Sam concentrated on remembering the details of the vision: bloody walls, a broken body crumpled on the floor, an address - 644 Robins Lane.
Robins Lane, Robins Lane, Robins Lane, all the little birdies on Jaybird Street, love to hear the robin go tweet, tweet, tweet.
Dean would be there in moment with meds and ice to help dull the resulting migraine. He had to remember to tell him when he came.

Rock, rock, a rock was digging him in the back, grating on his ribs. Breathe, breathe, breathe, that's what he needed to do. That's what Dean told him to do. When he gave him the meds and wrapped the ice around his head. Dean always knew what to do.

Go, he had to go. No, wait, that wasn't right. Dean would come, then they would go. Go, go, go where?

The trees swayed above him. The sun, low in the sky, played peek-a-boo through the leaves. No, go, no, this wasn't right, wasn't right at all. Dean, Dean, where was Dean?

Sam was going to Dean. That's want he'd been doing. Dean didn't know he was here, didn't know he'd had a vision. Okay, up, up, up get going, remember, remember the address.

Sam rolled to his left, tried to get on all fours, ready to hoist himself up, when a stab of pain pierced his right side. He groaned as he stretched his arm back, gingerly touching his back. His hand came back bloody, blood dripping down the walls. No, no, that was the vision, this was here, this was now, and his hand was covered in blood from his back.
A cool wind kicked up as the sun sunk lower in the sky. He had to get up, get back to Dean. Where was Dean? Right, right, okay, in the motel room, waiting for Sam. Gotta get back there.

Sam could do this, he could. The migraine wasn't blinding him yet, he could pushdragpull himself over to the tree. He could use it to lever himself up. He could remember the vision, he could get back to Dean.

The sun was glinting off the building. Dammit, Sam had been taking a short cut through an alley back to the motel. He'd been bringing back dinner, when the vision had struck. He was so close to Dean, to home. If only he could get up.

"Sam?! Sam you answer me right the hell now, or so help me god! Sam!"
Sam mustered his strength, knew yelling back would ratchet up the pain, "Dean!"
"Sam! Where are you? Sammy!"
"Down, down here, Dean, here." That last bit drained his reserves, but it was enough, as Dean crashed down the small hill towards Sam. Dean always came.
Dean's hands fluttered around Sam's face, his torso, "What the hell, Sammy? You hurt?"
"Back, back. Hit my back on a rock." Dean instantly patted his back down, finding the injury, "Its not even that bad, Sam. What happened, you trip over your ginormous feet?"
"Vision. Had a vision."

"Well, shit, man, c'mon. We'll get you cleaned up, sort all this out."
Sam did the best he could, but the combined agony of the migraine and the slowly bleeding puncture wound deep into his ribs didn't leave him all that coordinated. But Dean was there, with meds and ice and stitches and words.
"Okay, tell me what you saw, Sam"
Sam closed his eyes, "Bloody walls, a broken body crumpled on the floor, 644 Robins Lane."
