All of a sudden, Sam's not only terrified by the object of his phobia, he's also incredibly guilty. He keeps trying to expose himself to the object of his phobia when Dean's not looking, which makes him freaked out and exhausted and constantly afraid. He’s been out of water for what feels like more than a day, his tongue is fat, swollen, and dry in his mouth. He’s limping (new injury, permanent, you choose!) and still, he needs to rush, to hurry, to find some way to make himself run because the need to go as fast as he can in the hopes of catching up to the shadow he hopes might be Dean is almost as pressing as staying ahead of the sound behind him, the sound of something wet moving in the ruin.

And then everyone else freaks out as well because they are hunted by some monsters and then this guy who seems to know what he's doing is yelling about something in the dark.
