I have to wonder, though, if Cas is regretful, why didn't he insta-heal Sam?

I'm not sure if Cas even could do that before his upgrade following his first "death" at the end of the fourth season. My memory isn't perfect and I can't check the episodes right now, but I do remember that he didn't heal Dean after Alastair beat him up in On the Head of a Pin, so I assume he couldn't.
