The RL description of the character she's playing:

Sabina Spielrein (played by Keira Knightley) was dropped off at the Burghölzli Psychiatric Clinic in Zurich by her uncle and a medical police officer at 10.30pm on 17 August 1904. The wealthy Jewish-Russian 18-year-old was in Switzerland for a restorative sojourn that had failed to ease her troubled state of mind. She made a terrible scene - presumably not the first - at the luxury hotel where she was staying and her relatives gave up.

Her hospital records note that the 'patient laughs and cries in a strangely mixed, compulsive manner. Masses of tics; she rotates her head jerkily, sticks out her tongue, twitches her legs… Cannot stand people or noise.’ The notes are written by a newly qualified Dr Jung. He diagnosed her 'hysteric’.

she's playing it the way it's supposed to be played. obviously this is the performance cronenberg wanted from her. but people will criticize her no matter what.

one of the most common cures for hysteria was for the doctor to masturbate the woman to orgasm. this was how the vibrator was invented. some of the first vibrators were the size of massage tables. it made it so the doctor did not need a lot of "skill" to cure the patient. the things college teaches these days...
