Harry was naked in the end. In this particular scene, the real Harry rips off his clothes and dives into a freezing lake. But it was never specified how naked. So Ron could've just taken nigh-naked Harry in that context and envisioned him with a nearly nude Hermione.

Or Kloves was beating off to HG/HP fanfic and decided this would be a spectac idea.

In the books all that is mentioned is that Hermione pulls Harry in close and "their lips meet." THAT IS ALL.
