Because this is incredibly unhealthy? Young women need to know this is not a healthy way to live, I'm not shaming her I'm saying she needs help, and to conflate it to fat shaming is such a false equivalence

Just reverse the situation, does it sound okay to say “i’m nauseous by being able to see the belly rolls of this overweight person because it looks so unhealthy”? Nope it’s not. Over or underweight, no matter how unhealthy you think a person is, I think it’s simply not okay to say something like that. Eating disorders are literally disorders.
