He fell before, mixing up After and Before is very common when starting to learn English if you speak Spanish.

SAME is Sistema de Atencion Medica de Emergencia, in English it would be something like: "Emergency Medical Care System"

I got the impression the police/EMS were like trying to get into his room and that's when he fell, when they were all at the door to the room?

I haven't read any of the official sources though, it could be a translation error as you say!

Like someone's already said, SAME is the medical emergency system that we have in the city. Usually they send an ambulance with a driver and a doc on board.
It deals with emergencies that happen in the street/sidewalk, public transit, mass casualties situations, homeless people, etc. and they also coordinate with the police to provide medical attention when someone calls the cops, like in this case.
