Generally pretty difficult, yes. Besides it just being a really expensive lawsuit and not easy even in other jurisdictions, US defamation laws are designed to default more to allowing speech than even other places.

But then Samantha Markle was pretty much throwing shit at the wall, so even in a more permissive jurisdiction I doubt she'd have got far

It depends. Scamantha never had a legitimate complaint, her claim was pointless to begin with. She has one of Trump's lawyers because she's funded by the Heritage Foundation. They pay her to do this so they have material for fundraising from their racist supporters. They're also the ones suing to see Harry's immigration forms. It's all a game to them.

It's gone on for so long because the judge wanted to let it play out strictly by the book so she would have no grounds for appeal. This is the end of it, regardless of what she claims now.
