Anyway back in December, you probably remember participating in research for my science fair project. That project placed in the school science fair, and is going to the city fair! The city fair, however, is not as lax on the rules as my school is. Because I used human subjects, I need expressed consent of my participants before I enter in the city fair, or else I could be kicked out.
If you choose to give your consent, the terms are as follows:
1) I consent/assent to voluntarily participate in the research study.
2) I realize I am free to withdraw my consent and to withdraw from this study at any time without negative consequences.
3) I understand that my confidentiality will be placed at the highest value. My username will not be included in any public report, or on any display board. My username will only be shared in the event that science fair judges deem it necessary to prove the project's credibility.
4) I am the owner of the username which received this message.
