It's a small detail to grasp, but one Sanji's having a hard time jumping over. If it's been decades... then...
Maybe Kushi-san saw this guy from the back and didn't catch his age. Which would of course mean Miyabi's much older than him (why isn't that as much of a surprise as it could be?) Or they're two different people? Or she did meet a poncle and left the detail out]
