Ian is just a whole lot of "bwuh" in Sanji's eyes. So obviously not human, but far more empathetic than most of their species, and his cheeriness Sanji isn't sure to label as naive or just intriguing. Not quite a coward, either, and more than willing to take Sanji's punches.
But what really gets him is Ian sees Sanji for the type of man that he is, with more clarity than three meetings should allow. Sanji kicked him in the head and two comments later was referred to as a "nice guy". Obviously, he's not used to positive responses when he pile drives a head into the ground. Then he accidentally egged Ian into getting sick and all the tree-man really did was apologize and thank him for the water he brought. It's offsetting and reminds Sanji very much of Luffy, who can cut straight to the heart of the matter while still being an ignorant pain-in-the-ass.
And maybe that's why Sanji's so eerily patient with him, in his own way. He sees a bit of Luffy. He sees a bit of Usopp. He even sees a bit of Brook. It makes labeling Ian difficult because he's no friend, but he's not exactly an acquaintance, either. He’s… something. Maybe something to protect.
And for the record, Sanji doesn’t give a damn if Ian is immortal. He never wants to see the man injured or “killed”, if he can help it.
