To credit, when you go to upload your icon, you'll see this box
After you get your file... where it says "Keywords:" put in that box something like Credit: twistedenvy ... or just twistedenvy is fine. You could also put it in the Comment: part too. Some prefer that you put it there, but most prefer keywords I think. It doesn't really matter to me either way, but I prefer both.
