This fic has over 150,000 hits on A03 for a reason. It's an original novel. George Lestrade as a girl is fantastic. This Mycroft is actually likeable. Sherlock as an addict poised on the edge of self-destruction is too real and fragile and also arrogant as f*ck. It follows BBC canon, but has a hell of a lot else to recommend it. Read it even if you don't like genderswap, perhaps even if you don't like Holmeses. It's really that good.
