After rereading it with a clear head, and your little explanations, I can see MUCHO potential for this one, if you wanted to rewrite it. Wanna hear my suggestions? Ok. (lol)
You could do one of two things. One, you could make it a little longer (short CHAPTERED fic, maybe), making Zack one of Brian's patients, and giving them a little more time to get to know and interact with each other, then have Zack on his death bed, showing Brian what true happiness is, therefore still being his guardian angel, and saving him before he takes his last breath. That way, Brian falling in love with him is a little easier to...um..believe. (for lack of a better word)
OR, two, you can keep it like it is, with Brian never seeing Zacky before, but show how Zacky sees Brian, and instantly knows he needs 'saving', and SOON, since Zee IS on his deathbed and all. And instead of Brian falling hard for Zee at the end, just show that Brian HAS had an epiphany, and how truly greatful he is for meeting this beautiful stranger and for turning his life around.

Either way you go, I think a little more interaction between the two would be good. Maybe a week of daily visits (for option two) or even a month of daily visits (if you go with option one).

I truly believe this one could be BEAUTIFUL! And, I'm gonna go out on a limb here and say...if you even WANT to rewrite it, I would be more than happy to help! :-D

I seriously need to get going now tho. LOL! I'm gonna make myself late for work if I dont!

thanks for the suggestions!! hmmm...im deff thinking about re-writing it, and about taking up your offer!! that would be great!! i actually wasn't planning it to be a chaptered fic, but you seriously have pointed out some very important things. its hard to write somthing that makes sense if you only get 2 hours of sleep and stay up until 7:30 in the a.m. to finish it!! lol!! i know, i should'nt be giving out the excuses, but i WAS like half-dead when i wrote it, so you can see why i may have forgot some important things!! lmao!!
but, i deff wouldnt mind you helpin' me out with it(if i really do end up re-writing it.)because i need someone to tell me what may need to be added and taken out. id really love that if you would!!
