Depends... If I'm in the office, I would do my best to get to the ground floor so I can go on a killing spree. From home, I would stay home and kill the zombies as they come to protect the ones I value the most.
2.) What weapons would you choose?
A pair of desert eagles... or semi-automatics. Snipers are much preferred though reload time takes forever, so it's not suitable for an apocalypse. A nice katana would come in handy as well when the bullets are gone... and a magnum revolver as back up because those never jam like the recent gun models do.
3.) What supplies would you choose?
Lots of water, distilled, since I have amoeba. Possibly crackers. Milk and potatoes would be preferred since those two will give you all the vitamins and minerals you need, but in terms of storage for long term... I think crackers and beef jerky will do the trick.
Also, first aid, bandages, and a large number of bullets. Additional guns too, as many as I can carry. Light weight, as you need to run a lot.
4.) What mode of transport would you choose?
A nice, 400cc, motorbike. Any brand will do, as long as it's fast and easy to handle so I can still shoot/slice with one hand while steering with the other.
5.) What is your general battle plan?
Kill them before they kill me. Wipe out as many as I can to keep the zombies from spreading. Protect whoever is uninfected and teach the ones who are old enough how to shoot a gun and aim for the head.
6.) Any other notes?
Be a nuisance and I'll shoot you too. *^_~*

And I'd pick a car, because zombies can claw on you so easily when you're on a motorbike. ._. And I don't know how to drive a motorcycle. XD
General battle plan for me: stay alive, get to a safe place, save as many people as I can.
