regarding alec being chosen as max's breeding partner, i absolutely think that he was chosen as her partner because of his resemblance to ben - there had to have been all kinds of evidence connecting max to his death (her fingerprints would be on his neck, for starters, and the broken leg would be evidence that he'd been in a fight), and i think it was part of renfro's attempts to break max's spirits and have her become a true blue soldier, but it's obvious that he doesn't know that that was part of why he was chosen (if i'm correct and that it was). i don't think that renfro had much of an input on the situation aside from that until he reported to her that max was trying to escape. there's also the possibility that renfro chose to put them together because they were the "rebels" - max with her determination to be free again, alec with what had happened with the berrisford mission and the deals he had running with the guards. him lying to cover for her could have been a way of sticking it to renfro - i doubt he was a big fan of hers, either, with the amount of times she put him through psy-ops.
zack had been brainwashed to hell and back prior to the events of some assembly required - he didn't even recognize max until she showed him her barcode, so i'm not surprised he didn't recognize alec as ben - even if he had showed him his barcode, he wouldn't recognize it, because 494 wasn't a part of his unit. i figure that he lost touch with ben prior to pollo loco (and i won't get into how creepy it is that he was keeping such close tabs on max and not letting her have contact with the rest of the escaped x5s while ben was becoming a serial killer and tinga was allowed to have a family).
sam might have recognized alec but she wouldn't have any connection with him like max did with her unit. i'd have to watch the episode again to see how she reacted to seeing him, but if i recall correctly, she seemed to brush him off/not acknowledge him.
