Johari/Nohari links?

a long while ago (a year or more) there was a johari/nohari meme making its way around LJ, with links to actually making johari windows for oneself.

can anyone hook me up with the meme again, or better yet, to a legit johari-creation link that *isn't* an LJ meme, prettyprettyplease?
