First, I'll start off with with an apology for taking so long.  I mean seriously, this took a very long time to write for multiple reasons. I've actually written this tutorial like five times but kept losing it! UGH.  But anyway, here it is!
First off, I started by cropping the image into the size I wanted. Here is a similarly colored screencap because I don't have the original :(

--->

I went from the first (the original form of the cap - why the hell is it so dark!?) to the second by adjusting the channel mixer and color balance levels.

Color Balance levels:

Channel Mixer:

Then, this weird green thing:

Then, I added my brightness/contrast and curves layers:

to yield:

I then added textures and a duplicate of the previous steps stamped with different settings to start getting the color of the icon.

screen @ 83 and soft light @ 100 x2 respectively

MORE BRIGHTNESS/CONTRAST!

YASS, it's coming together! I then colored Hook's face more peachy to bring the color out.

+ peach coloring set on overlay @100 --->

Then I added a texture that looks to be absolutelybatty's (this one perhaps?) set to soft light @ 100

+
--->

Then, I stamped all layers and set the stamped layer to hard light @ 53%

+ hard light stamped layer ---->

Then I stamped all the layers (yes, I love stamping - it's control-shift-alt-e on a PC and even though I have a mac, I can't remember the keystrokes for PS on mac) and tilited it

+ tilt ---->

And yeah, the icon is pretty much done. Just some last minute touches!

I added a warming filter (85) at 25% density photo filter and erased a lot of it.  Then, I stamped and sharpened it.

+ photo filter + sharpen ---->

And then I stamped one more time, set it to softlight and erased up the face a little just to bring out the colors

+ stamped layer set to soft light ---->

AND WE ARE DONE!!!
