It's fear that keeps him going. That one day his secret double life -- will be discovered.
By day Ianto is serving them coffee, fake smiles and cleaning messes and by night he sits next to her -- witnessing the woman he loved (loves - he refuses to believe that it's not her anymore. Maybe that's a lie though, and he's just taught himself to believe -- like they believe he's got nothing to hide, and nothing to offer them.) suffer. If anyone notices how tired he looks sometimes, nothing is ever said. They're too wrapped up in their own lives, this job to notice much.
He's not sure sometimes, but it's too late to give up now. If he went to Jack and told him, it would reveal his betrayal -- he would surely lose her, this life - all his memories. He's worked too hard to let it all go.
So he keeps going (lying) -- Ianto Jones does, and well.
