Also regarding Dean not having laughed that hard in years, maybe it could also be chalked up to, like you mentioned, the distance/separation in S4...but then before that he was in Hell and it felt like 40 years. So even though he might know that it wasn't, and even though it's not brought up much in S5, I can't imagine that experience is gone from his mind.

I only wish they'd been apart longer. I think Sam snapped back to "gotta-hunt" mode too fast. Like a little kid who insists he doesn't have to go to the bathroom right before a long trip, and five minutes after they hit the road, he's wiggling in the back seat going "Ma, I gotta pee!". I would have liked to have seen them apart for at least two more episodes.
