I think I may have some theories to match your answers there *S*
1) Probably not meant to be a safe place so much as 'this won't kill you immediately' place. Personally I feel bad for Dean. He'll never voluntarily step on a plane again.
2) I'm thinking it's God. Maybe the angels (Zachariah) think they're ruining the show, but if Kripke sticks to his biblical lore, God's omnicient. It's not out of his power to recreate an angel. After all, if Zach could make Sam's lungs disappear by just saying so, God can bring back Castiel.
3) Dean being the Sword reminds me of a manga / anime called Angel Sanctuary http://en.wikipedia.org/wiki/Angel_Sanctuary (for gods sake, stay away from the anime! *shudder*). The sword in question was always by his owner, Alexiel, both of the reincarnated as humans together throughout history since Alexiel was banished to earth from heaven. Good read, very edgy and controversial, and the artwork by Kaori Yuki is gorgeous.
I also loved the fangirl. I both laughed and hid my face (watching with the family tonight) at her writing fanfiction when we first see her. Just...wow. *L*
I also agree that Sam needs to stop looking for forgiveness and get on the ball here. I think he's still in shock that what he thought was right will probably end the world and I hope his head gets back in the game soon. I think at this point it would be out of character for the Mea Culpa routine, but hopefully he'll grow out of it :P

I kind of wish the fangirl mentioned she wrote Sam/Dean. I bet they would've just loved that.
