I've read a few explanations of what was wrong with the ending, and it's enough to get me worried about Dragon Age III. I can't believe the same company that gave us Bhelen Aeducan and the Architect could be so tone-deaf.
So far as I understand, the original writer/director/whoever in charge of the Mass Effect franchise was booted off the job near the end of ME3 development. The guy they put in his place wanted to have his own mark on the game, so wrote/directed/whatevered his own ending. It was not the original planned by the original guy.

The Extended End was free DLC - so if thats their lesson, they missed off an important $ sign.
The Extended End also comes with a "non-apology statement" about how much Bioware values shared creativity with its player base.
Still, I was also greatly amused by the leaked quotes from during ME1 and ME2 development. They are on the internet and worth a read. One of them is a game developer's classic and applies to more than computer games:
"If we could give people a magic hat that would create for them the exact RPG experience they’d always dreamed of, they’d complain about the color of the hat.”
