1) Melchoir was the other angel who was actively torturing Cas, the one who'd studied Dean so closely.
2) Samael never used a blade on him, although he watched the whole time and was the one who created the restraint cuffs. He'd actually been one of Cas' lieutenants, and that was how they'd managed to grab him in the first place.
3) Casting: Samael would be played by Sam Witwer and Melchoir by Zeljko Ivanek. And Josiah looked like a thinner Jorge Garcia.
(Torn's probably the easiest fic for this meme. I had figure out who those three were and what they were about, and obviously none of that would make it in.)
