Sam made Lucifer promise not to hurt Dean before saying yes. So Lucifer doesn’t. Dean is the only one left alive, and he’s not quite human anymore. Just him and Lucifer, and Lucifer always keeps his promises.

I know this is half a year late, but I've filled this prompt

Lucifer leaves Sam before Dean, Cas, and everyone else can get killed. Sam is a mess - hurt, ill, completely broken, unresposnive, only speaking in fragments of Enochian that only Cas can make sense of. While everyone hates him and wants to hurt or kill him, and Dean can't bring himself to deal with his brother yet also can't let him go, Cas takes care of Sam and grows increasingly protective of him as he firgures out more and more what torture and mind games Lucifer used to make him say yes in the first place.
