Done in CS 2, but should translate fairly easily.
Requested anomalously...

I don't have the psd saved so I'll get it as close as possible to the original.

Make a graphic like this:

I started out with this texture from dearest




After deciding what picture I wanted to use, I cut and pasted it onto the texture. I added a 13px stroke in white (layer>>layerstyle>>stroke) I also added a black and white gradient map adjustment layer to desaturate the picture (layer>>new adjustment layer>>gradient map) To do the gradient map, make sure that your background color is black and that your foreground color is white. After you have the adjustment layer added, you can adjust the black and white amounts according to your picture







Now after making sure that I had everything the way I wanted, I merges my layers and then opened this texture from brasaremean





After resizing this down a bit since the original is huge, I drug the merged layers of the other texture and picture of Johnny over on top of this.





Alright, next I added some text. I used The Times New Roman font in all caps. I made the color white and added a 1px stroke around the text just to make it stand out a bit more from the background. Then just above it, I added some more text, but in black this time and no stroke around the text.




The text looks pretty good, but to give it a little something extra, I duplicated each layer of text, flipped it horizontally (edit>>transform>>flip horizontal) and set the blend mode to Softlight and moved the layer a little bit. I then also duplicated the original 'through my eyes' layer one more time, moved it down a bit and changed the color to black.




I wanted to add one more smaller picture so after choosing the one that I wanted, I pasted on a layer between the two texture layers. I wanted it to be kind of above the text so tried to get it approximately in that area. Of course since it is between the 2 texture layers, I can't see the picture so now I needed to make a space so that the picture is visible. I just added a layer mask to the layer that has the original texture and picture of Johnny that I started out with and used a rounded rectangle brush that I just made using the shape tool.

At this time I also set the picture blend mode to Softligh 100% and added a black and white gradient map adjustment layer and a brightness/contrast adjustment layer.




It looks icky....too green and glaring. So to fix that, on a layer just above the green texture, I added a gradient map adjustment layer using this gradient that I think is from meleada but correct me if I'm wrong.








That looks much better :)

Just a few finishing touches left.

The first thing I did was select the layer with the original texture and picture on it and add a 5px stroke in white. I then made a new layer and took the pen tool. Make sure that you have the paths option selected at the top.



Then making sure that you are inside the part that is stroked, start at one corner of the picture and click to add an anchor point. Then go to the next corner and click again while holding shift down to get a straight line, then to the next corner and again shift/click and then finally shift/click on the original anchor point to close the path.

Now go to your brush tool and select one of the default round brushes at about 5px. Then go back to the pen tool and right click anywhere on the path that you just made and select stroke path. Do not tick the simulate pressure box. Click OK. If you like how it looks and don't want to adjust the brush size anything, then right click on the path again and select 'delete path'. Other wise just hit edit>>undo and adjust it until you like how it looks.




Added Johnny's name using the Carpenter font at 60pt in black. And then to finish the graphic off, I added this texture from meleada





The blend mode is Softlight 75%. On the smaller picture of Johnny, the texture was too much so I took the eraser tool and using the same rectangel brush that I used earlier, I erased that part of the texture and then went to edit>>fade eraser and put it at about 50% so that it had some of the texture, but it didn't look over bearing.

And that's it :)
