Some hospitals making a change to openly admitting fault & apologizing for mistakes. And patients then tend not to sue. Yay honesty, good customer relations, and the ability to fix procedural problems instead of conceal them! Thanks for the link, meowse.

Speaking of that sort of quality control improvement, Here's an article on the Toyota Production System, in which the whole assembly line is stopped if there's a problem. Believe it or not, it leads to much higher efficiency.
