Dororo is an Osamu Tezuka manga from the 60s, and the basis for the 2007 movie that is near and dear to the hearts of several people reading this right now.

Do you know where Tezuka has real fun with the gender topic? MW. The story is about the priest Garai, his lifelong connection to - banker by day serial killer by night - Michio Yuki, and a chemical weapon called MW.

I keep meaning to finish this--I picked it up last Otakon at the Vertical booth and read half of it on the train back home, then kept getting distracted from it once I was home with my computer and video games (as I do). I've found it very enjoyable in a mostly lighthearted but now and then poignant way, despite, as you say, the occasional confusion. (As things stand, I'm shortly after the gender revelation, I think.)
