I was just thinking, that maybe the reason Italy couldn't enjoy it at first was because of his problem with phimosis (which makes sex painful). I'm surprised I still haven't found any fanfic that at least mentions it. Oh well, there's always the kink meme, I guess ;)

i also wanted to insert the phimosis somewhere, but it's not something you can just mention and then ignore (though i did write that germany 'pushes the foreskin away' at one point), besides, i think unless you centre your entire fic on it, it takes something away from actual pr0n. i wikied it and... urgh. it doesn't look good at all *shivers*

there were a few requests for phimosis but they didn't get filled. maybe because despite it being interesting, it's not 'sexy'. *shrugs*

In fact, it was the 'pushing the foreskin' moment that made me remember about Italy's phimosis in the first place. I understand that people prefer avoiding writing about it because of the many reasons you listed. I guess I'm still surprised that I have yet to find a fic that at least mentions it :C
One day I had this idea for a fail!sex and fluff fic: Germany and Italy finally have sex for the first time, and soon Germany can't help but notice Italy's problem (which results in the most embarrassing scene ever). Germany then instantly goes into a caring-boyfrined mode and tries to convince Italy to go to the hospital with him to find a solution. After many crying!Italy moments he finally convinces him to take the surgery (insert tons of fluff and reassuring!Germany here). Then after some time they can have the most awesome and epic sex you've ever seen! :D

ooh, that's such a cute idea for a fic :3 (though germany knows of italy's problem already, since there's a strip where naked italy comes on germany showering and notices their... are different :P

i guess germany would first want to try out with masturbation. as wiki puts it, constant masturbation by pushing the foreskin back with force should help it retracting and you don't need to operate. XD)
