They're usually put over graves to mark them- graves where the dead are laid to rest. Each name belongs to the one who is burried there.

Sometimes. There are people that burn the bodies instead, or simply place them in a box above ground in a building made of stone.

That's what used to happen- people think it so wrong now, to leave a body out in the open, to rot and decay and be picked out where someone can come upon it.
