P.S. -I apologize for the quality of the photo but it was shot with my cell phone camera (which sucks).
Yes that is a Meritorious Service Award for a "General Evan Lorne", it presumably appeared in the episode "The Last Man" (Season 4 finale episode) but I couldn't find any shots of it in his office at the SGC.
The closest I came was this:

It however is clearly not either, because the seals/text styles are wrong on both.
