Every fictional world ever thought of or created becomes a plane, separate from each other plane. There are theories that if the creator of a world (plane) changes something about their creation that either the plane already created changes to these new things or an entirely separate plane is created with the changes. Ian Jeff Pat and Erica are stuck in all these different planes, and need to get back home to their plane. Inter planar travel is near impossible and only a select few have mastered it, and even fewer than them know how to travel to certain planes, the rest can only travel at random to other planes.
