I like that you're including more information than just author and title. Just a question: since a lot of authors don't provide summaries, do you want us to leave that blank? (I think making up our own would be... problematic.)
And question #2: do you need to weed out duplicates manually with this form? If so, could you keep this entry up to date on which authors/posting dates are already covered? Would save you time and would also prevent multiple people from submitting the same stories. (Yes, yes, I guess it would make sense if people submitted their own stories first, but I was thinking more along the lines of submitting stuff by authors that don't have a tag and aren't around anymore, thus likely to be overlooked.)
Annoying question #3: on LJ, if we link to the first chapter, a lot of times there is no easy way to find subsequent chapters because many authors don't take the time to go back and edit in a "next chapter" link (which, hello writers, this would be super useful, especially if you post directly to the comm :)).
