So, Dean, let's recap. First you told Sam that you wouldn't touch him once you left the hosptial. Then, you told him that he'd been touching you without your permission, that you never actually liked it. And to top it off, you threatened to leave him.

He is an idiot, but really, he's totally overwhelmed both by the situation, Sam's illness, and his own feelings. Mostly the latter, I suspect. : D
