At this point Dean is still taking all of his meds, right? (Because he says tomorrow he and Sam will start cutting down on them.) He's been thinking really clearly compared to how he was reacting to everything at the beginning when his mind was foggy on the same pills. I'm thinking either he adjusted to the meds pretty quickly (in which case they're actually heling him and he should keep taking them) or he's already started cutting down on his doses.

The way I planned it was that Dr. Logan pretty early on indicates that they should discontinue Dean's Thorazine, and then they put him on Zoloft, which is a pretty mild antidepressant, instead. (It's in the first chapter, I think.)

So my feeling was that, yeah, he would become more coherent in his mind on Zoloft rather than Thorazine, and yet, still, he's a tad muddled which is why he doesn't just escape the first chance he gets. (He's on two other meds, both are mild and of a low dosage, lowered, I expect, when they lowered the Thorazine.)
