1. He's torn between two. One being when he saw Ariadne for the first time - everyone go awwww. Now he was only nine when the other happened. Those last moments before his mom got in the car to get him medicine and was killed by joyriding teens.
2. First response: Nothing! He doesn't see much of anything he's done as deviant. He's made book for the mob, had sex in military uniform in dreams, and slept with his best friend from childhood when they were still in their teens. Likely would have to go with the fact that in the time between Mal's death and the Fischer case he made a regular habit of picking up rather seedy rent boys, using them and leaving without saying much of a single word.

1. No. Not even before his current relationship in Complications verse. He tends to want to put a lot of focus on one person, on giving and receiving pleasure and nearly impossible to do that in a threesome. 1. I think that depends on the verse. In Complications he has other ties. Given I've tied it into his father and his lost from the past, I think it would be a double edge sword. He might see it as a chance to move on, to find something fitting of his time now, the man he's become rather than the one he was. Of course, he might always question just where he is and which is which because of starting over.
2. For a drive. The Phantom brings him peace and he would almost always dress, hat and riding gloves and the Phantom. Another time and place in his head.
3. Irrational, no. He says if he has a fear for it, it isn't irrational. Getting lost is not an irrational fear. Not just in Limbo but, really, anywhere. He's big on directions and such.
4. It depends on his mood. He prefers things to be one on one, not gang bangs or huge orgies. It's how he approaches sex and isn't one to want things to seem rushed, hurried, pushed or harried. Prefers it, most of the time, to have a build up. Not needing an entire plot but something with kissing, petting, toying with one another especially. Minor acts of denial can get him going.
