I suspect it stops at '99' because of space issues (three digits or more aren't going to fit in the little orange 'there's something unread!' indicator circle -- it's currently sized for just two digits). But having an inaccurate number displayed is somewhat unhelpful, so I'm sure something can be adjusted one way or the other to show the real number. :)

yoksel has made a quick tutorial on how custom styles can be edited to include this option at http://yoksel.github.io/lj-tips/add-comment-subj-to-custom-layer/ although we are still trying to determine if we can change the way the options page is generated so that the setting always appears.

Both responses are in part completely confusing, but I was able to solve the problem by switching customized comment pages off, saving, setting the subject line on default comment pages to on, saving, and then switching the default comment pages off again. Presto, for no logical reason whatsoever, the customized comment pages *finally* have subject line space when making a new comment.
Also, I had no clue that only Plus or above accounts could even have custom comment pages in themes. When bringing forth such "improvements" maybe also "improve" the "dumb" (support request answerer's word, not mine) layout/theme editing pages.
