There's no reason for either paracetamol (or similar painkillers) or antibiotics to interact with a tetanus shot.

Your character could have an adverse reaction to the tetanus toxoid itself, however. It's possible to get sick from the attenuated form of any virus or toxin, especially if your immune system is already compromised.
