Does anybody know the eligibility criteria (with references to verify the information) that applies to couples seeking IVF [in-vitro fertilisation] in Australia, particularly NSW?

I've Googled and Google Scholar-ed "ivf eligibility criteria", "ivf eligibility criteria nsw" "ivf eligibility criteria couples", both with and without limiting the search to pages in Australia. Most of the stuff I've found is more related to the duty of the clinics, and legal issues surrounding the use of the gametes and embryos for research.
