Not a simple film, the Director's Cut explains the whole thing much better than the original film does
**Note - Spoilers for anybody who hasn't seen the film**
Remember Grandma Death's book? She talks about wormholes in space-time that can be used to travel through time, right?
Well at the end of the film Donnie's girlfriend gets hit by the car driven by frank. Donnie sees the massive wormhole storm thing in the sky and realises that he can travel back in time.
Right, then of course the film gets reverted back to the point where the Airplane engine smashes through Donnie's roof. He stays in bed this time, because he knows if he lives he will set in motion a combination of events that will end with his girlfriend being killed in the car accident.
So to save her life he sacrifices his own because if he never meets her then she won't be there on that night and won't get hit by Frank.
Now this film contains a whole bunch of paradoxes and this is quite a genius part, because paradoxes are inherent in Time Travel.
One such paradox is - Near the beginning of the film Donnie's vision of Frank saves Donnie's life by taking him out of his room before he is crushed by the falling engine. If he went back in time nothing would have changed so this exact same thing should happen, right? But it doesn;t, why damnit, WHY?!
The reason is because Donnie has already made the decision that he will stay in bed to allow himself to die. Thus he won't meet his gf, won't go to Grandma Death's house, and won't shoot Frank in the eye. Thus Frank won't die and thus he would have no reason to appear to Donnie in a vision!
Confusing stuff eh? This is a very deep and confusing film with about a million layers to get through. Watch the Director's Cut, it'll explain much more
