I think upbringing might definitely have something to do with it; I know some guys who are my age (25) and were raised very differently, and as a result, some just see me as one of the guys and have no trouble treating me like they would another man, but others always have to open doors, pull out chairs, and make remarks such as "don't lift that, it's too heavy for a girl" and "if I offer to pay, will you clean my apartment? Your's always looks so clean because you're a girl."

I think that if we look at it in terms of upbringing, Sam has mentioned that he was alone with lots of women who were older than my (and I'm guessing your) parents' generation, so he obviously has a lot of respect for women as people, but he's also spent his formative years inundated with women who were very into housekeeping, the only one of which we know had a job having been a makeup saleslady, so I can definitely see him as being into the "separate but equal" mentality that so many guys who honestly see themselves as chivalrous and not bigoted get into.
