Sam's Lucifer is just a twisted manifestation of Sam's own self-hatred, Stockholm syndrome, repressed thoughts, and other assorted baggage (and Sam realizes this).
Lucifer is for-real Tyler Durden in that sometimes, Sam can't tell if he's the actor or the observer.
I hope we see these expressed in canon more, because they're awesome and scary (as here).
Loved the car scene (with the subtle hinting as to who was really doing what), the checking-in-to-see Cas bit ("I’d recommend that you gentlemen give him at least a week before you try to interrogate him . . ." "When would we be able to just make sure that he’s all right?"), the marvelously tense bedside scene with Bobby and Sam running interference, Dean and Cas's partial reconciliation, and the note of hope at the end.
Sam understanding Enochian has been bouncing around for a while; it makes perfect sense, even though we've never seen evidence (yet).

I also agree that Sam speaking Enochian makes sense. It's been my head-canon for quite some time, and I really want it to become *actual* canon.
