I noticed this phenomenon when the republican "revolution" took place and they took control of Congress and the White House. The ultra right wing and divisive issues that were campaigned on and that ultimately allowed the GOP to take control allowed those like Limbaugh and O'Reilly to flourish.

They (right wing media) noticed that you didn't have to be reasonable, you could be sensational and flat out lie to get your pin-headed points across.

What this really all boils down to is this: Remember the bullies on the playground during grade school? They weren't the smartest, but they could hurt you with their fists. If they didn't like you, they just knock you down. Ask them to play a game of chess with you and you'd likely clean their clocks.

The tactics of the GOP overthrow just gave permission to the fools like Limbaugh to say whatever they want, even if it is swill.
