Well the lab explosion was an accident thought Catherine blamed herself for a bit, not sure she still does.

The other was me as a CSI, and couldn't just sit and watch a man get beaten possibly to death.

Well Catherine placed an unknown substance under the hood. Standard procedure, she just didn't think of checking the heat sourse that always got knocked on accidently. As I was processing something interacted with it and BOOM! I went through glass, got burn scars on my back and neck.
