Without Dean, Sam's hallucinations are worse. That's not why he comes back.

Sam still goes jogging, he's still working on dealing. Dean stalks him to the town he's in and joins his morning run. And the next day. And the next. Eventually they start talking. They are very sweaty, and this is an important element.

Sam's off-balance, not sure how he sees Dean right now or how Dean sees him. So he's not so quick to realize that maybe this Dean he's talking to isn't the real one. Once real!Dean gets into the conversation, though, Sam knows that this one is his brother.

Sam went the wrong way when he stormed away from Dean on the dock and has to awkwardly turn around at the water's edge and walk back past the car. Can he bring himself to walk past sadfaced Dean sitting in the unmoving car!
