The scar represents the mutual angst and hurt of Arthur and Alfred, YET TO BE HEALED. It's as plain as the noses on their faces yet they both avoid the issue so the scar won't fade. ONLY WHEN THEY MAN UP WILL TRUE LOVE HAPPEN.

Interesting, very very interesting. However, I believe the scar is, in fact, a representation of the unity between the pair. Both received matching scars in the midst of a vicious gang fight (their suits clearly imply that they're gang members) while protecting each other. They now wear them proudly to celebrate their connection, and a symbol of their willingness to give their lives for each other.

The scar may represent a connection, but the connection is only that that Alfred is a materialised facet of a part of Arthur's mind. Arthur is still stuck in the past, so his mind!Alfred still hast he appearance of revolutionarywar!Alfred with the blue ribbon representing the blue uniforms. However, he has recently managed to begin to overcome his past with Alfred and their relationship is bettering. He is beginning to recognise that the Alfred in his mind is no longer the Alfred that of today. The matching scar shows that he indeed, recognises it to be in his mind and his perceptions (as it is only Arthur that has the scar). The reverse Nantucket is only one of the discrepancies that appear to show that it is indeed, a product of his imagination as he accepts the real Alfred.

Adding onto that, revolutionary!Alfred is leaning in towards Arthur in a way that supports that theory; he is subtly angled away from modern!Alfred and towards Arthur, suggesting that he is a bit of a 'guardian' for Arthur in an attempt to guide Arthur towards a reconciled relationship with modern!Alfred.
In the meantime, the modern!Alfred is still conflicted. He attempts to distance himself from Arthur, both physically and emotionally, while he struggles to sort out his feelings. Currently, his fiddling with his tie is his way of trying to not watch Arthur as he sexily runs his hand through his hair. Revolutionary!Alfred only smirks, knowing all too well the thoughts that are running through his future self's mind.
