I want some help

I've been searching this story for a loooonnngg time but, I can't find it

I can't remember the story that much but it's about..

Wolfram leaves Yuuri and he moved to live with humans. He changed his look into black-haired boy and lives with Adelbert (it seems.)

and Yuuri tried to make Wolfram back to him, so he sent the flowers and things to the cottage that Wolfram lives.

and one day Yuuri found him, but he didn't notice he's Wolfram....
