Here is the parental consent form for the Con-G Survival Tag & Cosplay Picnic! Please print it out and get your parent/guardian to sign it in order to participate in the game.

If you are under 18 and want to participate in the Survival Tag game, you must :
1. print out this form & have it completed by your parent/guardian
2. Bring it with you the day of the picnic
No exceptions!
Sorry, we cannot let you play if we do not have permission from your parent/guardian.
