A Question of Introductions

In "The Doctor, The Widow and the Wardrobe", the Doctor introduces himself as the "Caretaker", since he's trying to keep a low profile, seeing as he's supposed to be dead.

I can't remember - in later episodes, does he fall back into his old habit of introducing himself as the Doctor, or does he just not introduce himself?

I know that in "Hide", he didn't introduce himself, because my brother D, who hadn't seen Doctor Who in ten years, spent the first ten minutes of the episode thinking that the ghost-hunting professor was the Doctor, and the Doctor was the villain. (Strange but true)
So... for other episodes, do any of you recall whether or not the Doctor introduced himself as "the Doctor"?
