It wouldn't be too much a stretch of one's imagination to pretend Tamaki was female. Instead of becoming Host Club King, he- she would simply latch onto Kyouya and co., Renge-style, and "rule" as Queen.
Her relationship with Kyouya wouldn't change. He'd stick with her because she'd most likely inherit the Suoh family fortune, she was the second smartest person in his grade, after himself, and still dumb enough to be manipulated.
Of course, it'd be a done deal when Renge actually appeared, and tried to latch onto Kyouya, too.
"No! Mommy forbids it! Mommy and Daddy is a match!"
