Below is a copy/paste from the IMM5256E document (which explains, among the other things, how to fill in the visa application form). Quotes are in italics, my comments are below.

Annulled Marriage: This is a marriage that is legally declared invalid. An annulment can also be a declaration by the Catholic Church that the marital union did not have a binding force.
I thought that Canada is a secular country that does not prefer or select one religion over another on the government level.
Dates (From - To) for which you were in the relationship with your previous spouse/common-law partner.
Even if it was like 15 years ago, I guess I am supposed to remember the exact dates. Oh my.
